Personally, I have never broken one New Year's Resolution. That's because I don't make them. If there is something about myself I want to change then I should probably be making these changes throughout the year as I'm discovering them. These would include exercise and eating healthy. These resolutions I make and break all the time though.

My resolution for 2012 is become less bitter and jaded. That's kind of a nebulous goal, but it's the sum of all my flaws and problems, so I figure it's a good overall goal that can be approached from a number of different angles.
